Please note that the PPRA WILL NOT process any new applications for the issuing of a Fidelity Fund Certificate (FFC) without the following documentation:
1. Completed Application form – template below
Plus, either
- Letter of Employment – Non-Principal – template below OR
- Letter of Employment – Intern – template below
If your application does not include ALL of the above documentation, you will be notified, HOWEVER the application will not be processed UNTIL such time when all the required documents have been submitted to us.
FURTHERMORE, the time to issue your FFC will start afresh if there is missing or incorrect documentation, this is in terms of Section 49(2) of the PPA.
